Equipment rental agreement and waiver template
A rental agreement and release to adapt for ski, bike or gear rentals. It is a starting point, not legal advice: waiver rules differ by state, and some states limit what a release can cover. Have a lawyer review it before you use it.

Renter agrees
I have inspected the equipment and received it in good condition, and I know how to use it.
I will return it by the due date in the same condition, apart from normal wear, and pay the charges above for late return, damage or loss.
I will not lend the equipment to anyone not named on this agreement.
I understand skiing, cycling and similar activities carry risk of injury, and I accept those risks to the extent the law allows.
To the extent the law allows, I release the shop from claims for injury arising from the ordinary risks of the activity. This does not cover the shop’s gross negligence or anything the law does not allow a release to cover.
